As the parent/person with parental responsibility for the named child, I hereby consent to their participation in the WMCC meeting. I confirm that:

I have read and fully understand the "Acknowledgement of the Risks of Motorsport" provided, recognising the inherent dangers, including the risk of death or permanent disability.

My child has no physical, medical or mental conditions that would make their participation in competition or practice unsafe.

I acknowledge my responsibility to ensure that both my child and I have read and understand the ACU National Sporting Code, Standing Regulations, Supplementary Regulations, Final Instructions and this Entry Form, and that we will comply with them.

I consent to the use of photographs or video footage of my child by event officials for safety, accident investigation or promotional purposes.
